Jon Perenack, MD, DDS
Course Chair

Dr. Jon Perenack’s practice is almost solely devoted to facial cosmetic and reconstructive surgery. He has received board certification from both the American Board of Cosmetic Surgery in Facial Cosmetic Surgery and American Board of Oral and Maxillofacial Surgery. Dr. Perenack completed fellowship training at the prestigious Facial Plastic and Cosmetic Surgery Center in Texas in 2003. Previously, he completed his residency in Oral and Maxillofacial in New Orleans at LSU Health Sciences Center. Dr. Perenack was a full-time faculty member of the LSU Department of Oral and Maxillofacial Surgery from 2003 to 2015 and was the Director of Residency Training at LSU from 2005 to 2014. He was President of the American College of Oral and Maxillofacial Surgeons in 2017 and has chaired the Cosmetic Section of the AAOMS Parameters of Care from 2016 to present. While maintaining an associate adjunct clinical professorship with LSU OMS, Dr. Perenack is currently in full-time private practice in Baton Rouge Louisiana and offers a 1-year AACS-accredited Fellowship in Facial Cosmetic Surgery. Dr. Perenack has numerous journal and book chapter publications in the cosmetic field to his credit and has spoken on the subject internationally.

Manolis G. Manolakakis, DMD, FACS

Manolis Manolakakis, DMD, FACS, is a dual board-certified Oral Maxillofacial and facial cosmetic surgeon and the director at Advanced Facial Surgery in Shrewsbury, New Jersey, and New York City. He specializes in all aspects of facial cosmetic surgery.
He completed a residency in oral and maxillofacial surgery at Nassau University Medical Center in East Meadow, NY. He later completed fellowship training in cosmetic and reconstructive facial surgery at Baptist Memorial Hospital in Columbus, Mississippi, and Emory University in Atlanta Georgia Teaching has been such a big part of his career that he created an accredited Facial cosmetic surgery fellowship for oral Maxillofacial surgeons to get focused training in facial Cosmetic surgery He has lectured on local regional national and international stages regarding facial cosmetic surgery

Sami A. Nizam, II, MD, DMD

Dr. Sami A. Nizam II is a board-certified oral and maxillofacial surgeon with extensive training in both facial reconstruction and cosmetic surgery. Born in Daytona Beach, FL, he completed his undergraduate studies at the University of Florida and earned his DMD from Nova Southeastern University, graduating 4th in his class. He was recognized with the Horace Wells Award for Anesthesia and the American College of Dentists Ethics Award. Following dental school, Dr. Nizam pursued a one-year internship at LSU in New Orleans, where he developed a strong interest in post-traumatic facial reconstruction. He then entered the six-year integrated Oral and Maxillofacial Surgery residency at Rutgers University in Newark, NJ—a program known for its balanced training across medical and dental disciplines. While there, he earned his MD from Rutgers and completed a year of General Surgery at University Hospital in Newark. Dr. Nizam advanced his expertise by completing a fellowship in cosmetic surgery at The Cosmetic Surgery Center in Little Rock, Arkansas, under the mentorship of Dr. Rhys Branman. During this intensive program, he performed over 650 cosmetic procedures, gaining proficiency in a wide range of surgical techniques.

Since 2013, Dr. Nizam has maintained a private practice, blending his skills in oral surgery and cosmetic procedures to provide comprehensive care. His diverse background uniquely positions him at the intersection of aesthetics and function, offering patients thoughtful, personalized treatment plans rooted in both surgical precision and compassionate care.

Faisal A. Quereshy, MD, DDS, MBA, FACS

A native of Toronto, Canada, Dr. Quereshy is a board-certified oral and maxillofacial surgeon by the ABOMS and is a board-certified facial cosmetic surgeon by the American Board of Cosmetic Surgery (ABCS) and American Board of Facial Cosmetic Surgery (ABFCS). After graduating from dental school from the State University of New York (SUNY) School of Dental Medicine with a DDS degree in 1994, he completed a specialty training residency program in oral and maxillofacial surgery at the Case Western Reserve University with an integrated medical school curriculum, whereby he earned his MD (1997) and a one-year PGY General Surgery training. He then further advanced his surgical skills, completing a specialized fellowship program, accredited by the American Academy of Cosmetic Surgery (AACS) and CODA (Commission on Dental Accreditation), in Facial Cosmetic Surgery (2000). Most recently he graduated with an MBA degree from the Case Western Reserve University Weatherhead School of Management (2024).

Dr. Quereshy has both academic and private practice positions, where he practices full-scope OMS. He shares a community-based practice with his wife, Dr. Najia Usman (Endodontist) - The Visage Surgical Institute and V-Spa, in Medina, OH. Additionally, he is a full professor with tenure, and served as the residency program director (2002-20025) for the department of oral and maxillofacial surgery, at the Case School of Dental Medicine and University Hospitals Cleveland Medical Center in Cleveland, OH.

He holds surgical memberships at all local, state, and national levels: ACOMS as President (2024-2025); president for the American Board of Facial Cosmetic Surgery (ABFCS) 2022-2023; president of the Ohio Society of Oral & Maxillofacial Surgeons (2022-2023); has been an examiner (seven years) and section chair for the American Board of Oral and Maxillofacial Surgery (ABOMS 2011-2017); secretary of the American Board of Cosmetic Surgery (ABCS) 2010-2013; board of trustee for the American Academy of Cosmetic Surgery (AACS); and past member of the Ohio State Dental Board (2018-2024).

For the AAOMS, he is the caucus secretary District IV; current Chair of the Committee on Education and Training (CET); chair of the Special Committee in Facial Cosmetic Surgery (SCFCS) and Clinical Interest Group (CIG) Facial Cosmetic Surgery; as well as a member of the editorial board for the Journal of OMS, and Committee on Continuing Education and Professional Development (CCEPD). At the local level, past president of the Ohio Society of OMS; presidents of both local Cleveland Dental (2016-2017) and Medina Dental (2005-2006, 2024-2025) societies and a delegate for the Ohio Dental Association; ,and involved in the representation at the House of Delegates for both ODA and AAOMS that are elected political positions that represent dentistry and oral and maxillofacial surgery.

He has given numerous continuing education seminars and presentations internationally and locally in his specialized field and has published a number of scientific and clinical papers, as well as textbook chapters. He is very active in the above professional organizations devoted to the advancement in the field of facial cosmetic surgery, facial surgery, dental implant surgery, and craniofacial and non-syndromic orthognathic (jaw reconstructive) surgery.

He and his wife have four beautiful children, enjoy golf, and enjoy supporting all Cleveland professional sports teams.
